Printing a Blank Caregiver Profile Form

Some users may wish to print out a blank caregiver profile form.

Turn off pop-up blocker before trying to print forms in DAISEY.

To print a blank caregiver profile form:

  1. Navigate to the “Search Caregiver” grid.
  2. Click the printer icon next to the “Add Caregiver” button near the top-right corner of the screen.

*This process applies to printing any form in DAISEY.
