Quarterly/Mid-year/Annual Report

PURPOSE OF THIS REPORT: This report contains information required by funders in the state of Iowa, as well as data that programs can use for program planning, evaluation, and Quality Improvement activities.

WHAT THIS REPORT REPRESENTS

Data in this report comes from forms in DAISEY. This report should serve as a guide to quickly and easily identify demographic and service delivery data. Please refer to the FSSD Data Dictionary on the DAISEY Iowa website for additional guidance.

USING FILTERS

Parameters will be applied to each dashboard when modified. Users will only be able to access data according to their assignment in DAISEY. The following parameters can be used to change the data seen in the report:

Reporting Year controls the fiscal year.

Reporting Period controls the Quarter.

Organization Name controls the name of the FSSD organization in DAISEY.

Program Name controls the name of the FSSD program in DAISEY.

County Name controls the name of the County selected in the caregiver profile.

*Participants controls whether the report includes Current or Discharged families or both.

Target child controls whether the report includes target child, non-target children, or all children.

*This is based on whether or not the past discharge date is entered in the caregiver profile.

REVIEWING DATA

Some graphs or bars in this report may have Unknown or Invalid data.  To review Unknown or Invalid data in these reports, it will be necessary to review the underlying data to determine what data needs corrected. DEMOGRAPHICS DASHBOARDS

These dashboards represent caregiver profile information entered for primary caregivers who are enrolled during the selected reporting period.

EDINBURGH POSTNATAL DEPRESSION SCALE

Form: EPDS

Who is counted:

  • Primary Caregivers who enrolled prenatally and have a child under the age of 12 months during the selected reporting period.

Details about this dashboard:

  • Data in this dashboard are taken from ALL completed EPDS in the selected reporting period. For example, if one EPDS had a Positive result, that is the screening that would be used, even if a later screen in the reporting period resulted in a Negative result.
  • This dashboard is a progression from the previous bar.

AGES & STAGES QUESTIONNAIRE-3

Form: Child Quarterly Report

Who is counted:

  • Primary Caregivers who are enrolled with age eligible children in the selected reporting period.

Details about this dashboard:

  • Data in this dashboard are taken from responses in the most recently completed child quarterly report.

AGES & STAGES QUESTIONNAIRE: SOCIAL EMOTIONAL

Form: ASQ:SE

Who is counted:

  • All children with primary caregivers who are enrolled and reach an age interval deadline (determined by Table 6.1 of ASQ:SE User’s Guide) during the date range selected.
  • Families have to be enrolled for at last 30 days leading up to the age interval deadline determined by Table 6.1 of ASQ:SE User’s Guide.

Details about this dashboard:

  • To determine due dates for children 0-24 months old, the Corrected Date of Birth is determined by using Gestational age at birth for children born at less than 37 weeks and their Date of Birth.

Report Function (based on ASQ-3 timing schedule):

  • Timing bar – Will first show up when window end is in the range AND today is later than window begin.

LIFE SKILLS PROGRESSION (LSP)

Form: LSP

Who is counted:

  • Enrolled Primary caregivers with a completed Initial LSP and a later Ongoing LSP that was completed in the selected reporting period.

PROTECTIVE FACTORS SURVEY (PFS)

Form: PFS

Who is counted:

Enrolled Primary caregivers with a completed Initial PFS and a later Ongoing PFS that was completed in the selected reporting period.
